Create a lifecycle engine bound to one machine + entity type + state port.

Construction-time invariant: if the machine declares ANY allowSelf edge but the port does not advertise serializesSelfTransitions, we throw — a status- only CAS can't tell a real self-transition from a no-op and would silently double-fire. (The default bindingPort sets the flag iff a versionColumn is configured.)
