Flatten a Conditions row + its current version into the engine's EngineCondition shape. Returns null if the row has no current version — those rows are excluded from the conditions table because the engine can't evaluate them.

Tolerant of nulls: forms stores conditions per-org, but a condition authored without name/description/module is still valid for evaluation. Empty string fallbacks mirror what src/server/rules/condition-repository.ts does.
