Expand a series template into occurrences from its DTSTART through through (INCLUSIVE — the horizon cursor semantics of §6.8: everything at or before materializedThrough has been processed).

Always expands from DTSTART, never from a cursor: idempotency rests on existing rows blocking regeneration (the (series_id, occurrence_at) unique), not on cursor arithmetic — and COUNT-bounded rules can only be evaluated from their start anyway. The result of the underlying iterator is collected ONCE into an array (rrule-temporal re-entry quirk).
