Optional, off-hot-path decision-audit hook. Called (best-effort) for every
decision that qualifies under the configured AuditLevel. A throwing
or rejecting hook NEVER fails the can() decision — the access service wraps
it. The default is a no-op; pass createTableDecisionSink() to persist to
the shipped rbac_decision_log table, or supply your own store / queue /
retention / PII-redaction implementation.
